Why GLP-1s Can Cause Hair Loss—And What You Can Actually Do About It

If you've started a GLP-1 medication and noticed more hair in your brush, your shower drain, or your pillowcase, you're not imagining it. Hair loss has become one of the most commonly reported side effects of GLP-1s outside of the gastrointestinal complaints—and for many users, it's one of the most distressing.

The research here is genuinely mixed. Some studies show protective effects for hair on GLP-1s, while others document hair loss as a clear adverse event.¹ Some researchers propose that GLP-1s might disrupt the hair growth cycle or accelerate androgenetic alopecia; others point to potential benefits from improved insulin sensitivity and better scalp blood flow. Despite all the theory, the mechanisms behind GLP-1 agonists and hair loss needs to be explored further.²

What we do know is that the people losing the most weight are the people most likely to lose hair—and we have a reasonable scientific picture of why.

The Weight Loss Connection

The Wegovy clinical trial on semaglutide offered one of the cleanest looks at this relationship. Among patients who lost more than 20% of their body weight, 5.3% experienced alopecia. Among those who lost less than 20%, the rate dropped to 2.5%.¹

That gradient is the most important thing to understand about GLP-1-related hair loss. The mechanism isn't well established yet, but most evidence points to rapid weight loss and resulting nutrient deficiencies as the primary drivers, not the drug itself acting directly on hair follicles. Faster, more dramatic weight loss creates more stress on the system, and hair pays the price.

The Three Pathways Most Likely Driving It

Researchers have identified three primary mechanisms that likely explain GLP-1-related hair loss.¹

1. Telogen effluvium from the stress of rapid weight loss

Telogen effluvium is the medical term for stress-induced shedding. Your hair follicles cycle between a growth phase (anagen) and a shedding phase (telogen). When the body experiences significant stress—and rapid weight loss qualifies—it deprioritizes non-essential functions to protect vital ones. Hair growth, while important to you, ranks low on your body's biological priority list compared to organ function.

The result: up to 50% of follicles can shift into the shedding phase, and visible hair loss typically begins about three months later. The effect is often more pronounced in women and older adults.³

This is also why GLP-1-related hair loss often shows up later than people expect—it's not happening when you start the medication, it's happening months after your body started registering the weight loss as a stress signal. This is due to the length of the hair cycle—it takes a few months for hair that has entered the shedding phase to actually release from the scalp.

2. Nutrient deficiencies

Hair has high nutritional demands—it requires a steady supply of specific nutrients to keep cycling properly. Because GLP-1s suppress appetite significantly, many users simply aren't eating enough to support healthy hair growth.

The nutrients most closely tied to hair health include:

Protein: Provides the amino acid building blocks for keratin, the structural protein that makes up your hair. Inadequate protein intake is one of the most common drivers of hair thinning.

Iron: Plays a critical role in hair follicle function. Low ferritin (the storage form of iron) is one of the most frequently identified culprits in unexplained shedding.

Zinc: Supports hair growth and repair.

B vitamins: Support keratin structure and overall follicle health.

Vitamin D: Associated with hair follicle cycling.

When appetite drops sharply, deficiencies in any of these can develop quickly—and hair can reflect these changes.

3. Hormonal changes

There is some emerging concern that semaglutide may cause hormonal changes that raise the risk of androgenetic alopecia (pattern hair loss), and that this effect could persist even after stopping the medication.¹ The data here is limited and needs more research before we can say anything definitive.

More generally, rapid weight loss itself can disrupt hormone production, since hormone synthesis can be deprioritized when the body senses that it's in a fasting or restricted state.

Do Some GLP-1s Cause More Hair Loss Than Others?

There is some evidence to suggest that certain GLP-1s cause more hair loss than others, but the picture is complicated by the fact that the drugs that cause more hair loss are often also the drugs that cause more weight loss.

An analysis of the FDA's adverse event reporting system from 2022 to 2023 found increased reporting odds of alopecia for semaglutide and tirzepatide, with no significant increase for liraglutide, dulaglutide, exenatide, or lixisenatide.⁴ But semaglutide and tirzepatide are also the most effective at driving weight loss, so it's unclear whether these drugs are intrinsically harder on hair or simply produce more of the rapid weight loss that triggers shedding.

A separate study found hair thinning was reported in almost 7% of people taking high-dose oral semaglutide, compared to less than 1% on placebo.⁵ Again, the more aggressive weight loss could be the real cause.

A broader pharmacovigilance analysis found that GLP-1 receptor agonists as a class were the most reported drug class for hair loss, with semaglutide, liraglutide, and dulaglutide being the three leading medications.⁶

The takeaway isn't that you should avoid the most effective drugs. It's that the more weight you're losing over a short period of time, the more proactively you need to protect your hair. It's also worth noting that the total amount of weight lost may not be the culprit–rather, it may be the rate at which that weight is lost, with faster weight loss putting more shock on your system and potentially driving more dramatic nutrient deficiencies.

GLP-1s May Support Hair Growth at the Cellular Level

As with skin, the GLP-1 hair story has a confounding layer. There are mechanisms by which these drugs may actually promote hair growth.¹

GLP-1 receptors are expressed around hair follicles. When activated, they trigger the MAPK/ERK signaling pathway, which drives the cellular proliferation and differentiation needed for follicle growth and maintenance. Beyond that direct effect, better glycemic control and reduced body weight improve blood flow and insulin sensitivity—both of which support healthy follicle function.¹

So GLP-1s are doing something paradoxical at the follicle: potentially supporting growth at the cellular level while simultaneously triggering shedding through the systemic stress of rapid weight loss and nutrient depletion. For many people losing significant weight quickly, the stress and nutritional pathways win out visibly.

The 90-Day Window

If there's one timing principle to understand about GLP-1-related hair loss, it's this: the first three months are the critical window.

This is when your body starts registering the stress of rapid weight loss and initiates the transition of follicles into the telogen phase.⁷ Shedding that goes unaddressed early in this window translates into avoidable density loss later. Acting in the first 90 days—through nutrition, supplementation, and topical support—gives follicles the best chance of staying in the growth phase.

This is also why many people don't see GLP-1-related shedding until months three or four. By the time it's visible, the underlying biological cascade had started months earlier.

If You Already Have Hair Thinning

If you're already experiencing hair loss and considering starting a GLP-1, the smartest first step is partnering with a dermatologist to diagnose the specific type of hair loss you have before you begin the medication. Tell your dermatologist about your plans. They can adjust your existing hair loss treatments before you start, monitor changes once you're on the medication, and intervene early if shedding accelerates.

This matters because different types of hair loss (telogen effluvium, androgenetic alopecia, autoimmune-driven loss) respond to different interventions. Layering a GLP-1 on top of undiagnosed hair loss is a setup for confusion about what's driving any changes you see.

How to Prevent GLP-1 Hair Loss

The good news is that most of the hair loss associated with GLP-1s is preventable—or at least significantly mitigable—with proactive intervention.

Pace your weight loss. Aggressive caloric restriction sends the strongest stress signal to your follicles. Losing weight more gradually minimizes the shock and gives your body time to adjust without triggering the telogen shift in such a large percentage of follicles.

Prioritize protein. Inadequate protein is one of the biggest drivers of hair thinning during weight loss, and it's also one of the easiest factors to control. Hit your daily protein goals consistently (1.2-1.6 grams per kilogram of adjusted body weight per day), and aim to distribute that protein across meals rather than backloading it.

Address micronutrients. Consult your doctor or a registered dietitian about supplementing with biotin, zinc, iron (specifically ferritin), vitamin D, and vitamin B12.⁸ Bloodwork can identify which deficiencies are most relevant for you—don't guess; test.

Act early. Don't wait until you're seeing significant shedding to start protecting your hair. The intervention window closes faster than most people realize.

The Bottom Line

GLP-1-related hair loss is real, common, and—for most people—preventable. So far, the data suggests that the drug isn't the primary villain here; the rapid weight loss it produces is. Stress on the system pushes follicles into the shedding phase, suppressed appetite drives the nutrient deficiencies that hair needs to keep cycling, and the result is shedding that typically peaks around three to four months after rapid weight loss initiates.

The interventions that work are the ones that address those root causes: pacing your weight loss when possible, hitting protein and micronutrient targets consistently, working with a dermatologist if you already have any hair concerns, and supporting your follicles topically through the critical early months and beyond.

You don't have to choose between your metabolic health and your hair. You just have to give your follicles the resources they need to stay in the growth phase while your body adjusts.
